Patchworkz LLC is a approved HCBS waiver provider for the BDS Medicaid Waiver program based in Indianapolis, Indiana. We specialize in providing person-centered support and care for individuals of all ages within the I/DD community. Our goal is to help each individual live meaningful, self-directed lives within their own community.

Patchworkz is specifically developed to provide services under the following Indiana Medicaid Waivers:

  • Community Integration & Habilitation (CIH) Waiver

  • Family Supports Waiver (FSW)

  • Money Follows the Person (CIH Transfer)

We provide a comprehensive range of supports tailored to each individual’s needs, including:

  • Residential Habilitation & Support (RHS): Assisting with daily living skills in a home setting.

  • Respite Services: Providing temporary relief for primary caregivers.

  • Community-Based Skill Development: Helping individuals build the skills necessary to navigate and participate in their local community.

  • Health Management Support: Leveraging our clinical leadership to ensure health and wellness needs are met.

  • Community Integration: Promoting meaningful participation in employment, relationships, and chosen activities.

  • Transportation: Provision to fully participate in social and recreational activities in the community as well as employment and/or volunteer opportunities.

Unlike many providers, Patchworkz features clinical leadership directly in our daily operations. Our expertise in wound care, therapy orders, and Durable Medical Equipment (DME) allows us to offer a more holistic, health-conscious approach to care. We are committed to high-quality standards and CQL-aligned service development.

We serve individuals with intellectual and developmental disabilities who have been approved for the CIH or FSW through the HCBS Waiver Program with the Bureau of Disabilities Services.

Patchworkz provide services under the following Indiana Medicaid Waivers:

Through DDARS approved selected contracting case management organizations and be enrolled by IHCP. To determined if you may qualify for services please contact the local BDS field services office near you.
